Since MYSA now has a new insurance carrier, please email the state office at office@mysa.org and request a Certificate of Insurance for each site you are using. This will allow us to make sure that all the information we give to the new carrier is correct. Your request must include:
Name of site
Name of property owner
Physical address of property
Also include address or fax number where you want certificates sent
