The Missouri Youth Soccer Association is moving forward to a paperless system by moving the registration process to the Got Soccer system.  The process of getting all our paper forms converted to electronic copies is time consuming and progressive.  Eventually MYSA will be almost 100% paperless.   We have re-worked most of the website to reflect these changes.  There are still some items that we are currently working on to get to the electronic system.   The forms you find this page are forms that are NOT in the Got Soccer system and still need to be accessed via the method that we have always in the past.

It is no longer necessary to use the "Out of State Permission form", "Dual Roster form" or the "Player Membership form".   The Out of State Permission and Membership form are created when you make a profile within the Got Soccer system.   In regards to the Dual Roster form please collect a email or hand written note from both coaches giving their permission for the Dual Rostering and attach that to the player paperwork or give to the registrars for uploading to the player profile.
